DISCHARGE PLANNING


Planning for your discharge may begin upon admission to the hospital to ensure continuity of care. The combined knowledge of nursing, social services, physical therapy, occupational therapy, and dietary personnel is available to help patients and their families identify post-discharge health and wellness needs. You, your physician, or your family may request discharge planning services by contacting the Discharge Planning Department or your nurse.

Discharge Times

Patients should plan to leave the hospital by 11:00AM on the day of their discharge. If you have any questions, please contact your nurse.

  • Please communicate your transportation plans with your nurse, or if assistance is needed.
  • Your nurse will give you instruction about your medications, diet, follow-up health care appointments reminders, and special directions. Telephone numbers to call for questions after you are home will be provided at this time.
  • If you need any special supplies, supportive home services, or home health or hospice services, our staff can assist you with those arrangements before you are discharged.
